If you are a prospective patient hoping to make your first appointment with Project Vida, please go to the “Pre-Registration” tab of this website to use the pre-registration portal.

If you are a returning patient or a patient who has used Project Vida services in the past and are hoping to look at other services: see your health records, get information and refills on your prescriptions, or get in contact with your provider for non-emergency purposes, among other functions, this is the portal for you.
