The goal of the Navigator is to access children, youth, and families in schools to School-Based Integrated Health care.
This position is for an individual who is passionate to serve the community with empathy to increase health knowledge and improve health outcomes through a range of activities such as outreach, education, navigation, advocacy and strategic partnerships with community-based organizations.
The HIV Care Manager is responsible for the engagement of new and existing patients with HIV with prevention and treatment services. The case manager will also identify those at risk, increase the number of patients tested for HIV, and link patients to treatment.
Assist payroll staff in preparing accurate and timely payrolls. Assist maintaining accurate, up to date, and accessible records.
The front staff contributes to Project Vida’s continued success by diligently executing the organization’s mission and promoting the organization’s culture and values. The front office provides services that include reception, scheduling, patient check-out, billing support, dental records, and administrative support.
After-school Specialist

Support students with (homework, school projects, tutoring, mentoring). Enhance extracurricular activities (arts and crafts, science projects, community service, leadership, sports, recreations, computers, technology, and health). Facilitate and monitor a wide variety of educational and recreational activities for participants with diverse groups.
Lead Teacher

Plans and implements activities designed to promote social, physical and intellectual growth of the children under her/his care. Documents developmental assessments for children in classroom using the Deveraux Early Assessment for Infants and Toddlers (DECA/ IT), Ages and Stages Questionnaire (ASQ-3), or Teaching Strategies’ “GOLD Assessment.” Implements curricula “Infants, Toddlers and Twos” and “Frog Street Pre-K” and develops weekly plans with daily lessons.
The care manager is responsible for providing and coordinating depression care for patients in the Integrated Behavioral Health program The Clinical Care Manager educates patients about depression and anxiety and their treatment, provides behavioral activation, monitors depressive symptoms and severity of anxiety in response to medication and/or psychotherapy using a structured instrument (e.g., the PHQ-9 and OASIS), works closely with the primary care provider, counselor(s) and psychiatrist(s) to revise the treatment plan when patients are not improving, and offers a brief course of counseling for depression (e.g., Cognitive Behavior Therapy (CBT) or Problem Solving Treatment in Primary Care (PST-PC).
