Diabetes Care Coordination
Our Diabetes Care Program seeks to support the retention and continuum care for patients with diabetes type II by providing: care follow-up, education, and referrals to needed social services, in order to reduce patients’ health disparities and address their social determinants of health. Chronic Care Coordinators (CCC’s )provide education, awareness and provide the necessary tools for adequate self-management and the prevention of complications.
Diabetes Self-Managment Education (DSMEP)
Accredited by Association of Diabetes Care & Education Specialists (ADCES)since 2012.
A free glucose meter is provided to every participant during the course, which consists of a two-hour weekly class for a period of eight weeks.

Support Groups
These support groups are designed to provide an ongoing process after 8 Diabetes classes are completed. These classes use the 8 Dimensions of Wellness by applying the model and become aware of the interconnection of each dimension and how it contributes to healthy living.
These 2-hour group sessions create a safe environment for participants to learn how to take control of their health by engaging discussion on health such as Diabetes, Hypertension, Pre-diabetes and obesity managment. The group provides support in different aspects such as: emotional, spiritual,intellectual, physical, occupational, social, financial, and environmental support, advise and encouragement for each participant.
