prevention & chronic care program
Project Vida Health Center’s Prevention & Chronic Care Program is the founding pillar of the Wellness and Prevention Program. Implemented for over 25 years, its focus is to provide services through Chronic Care Coordination (CCC) and the implementation of Outreach and health promotion strategies. This program is run by Promotoras (CHW) at all levels, from administration to education. We provide classes for Diabetes Care, Pre-Diabetes Prevention, Hyperlipidemia, Sexual Health & Reproductive Services, Healthy Cooking classes, Support groups, and physical activity, for the overall wellness of all patients suffering or at high risk of chronic diseases. We focus our services on reaching diverse backgrounds throughout El Paso's communities. Currently, this program has transformed from state-funded in its initial stages to self-sustained since 2012.
Project Vida prides itself on building relationships with other organizations that carry the same values, such as caring and helping the community. Currently, Project Vida sustains partnerships with El Consulado Mexicano, Diabetes Alliance, American Heart Association, El Paso Center for Diabetes, Paso del Norte Health Foundation, American Association of Diabetes Educators Specialists, American Diabetes Association, and our Facebook group, Bueno para Comer.
