Hypertension Care
Hypertension Care Program supports the retention and continuum care of patients with high blood pressure and those who are at risk of developing it by providing: care follow-up, education, a blood pressure monitor (only if diagnosed with hypertension), and referrals to needed social services, in order to reduce patient’s health disparities and address their social determinants of health. Participants will receive the Care Coordination and 5 one to one education interventions.

Eligibility
Patients of Project Vida Health Center diagnosed with hypertension 130/90, or being at risk of developing hypertension by the following conditions:
- Diabetes
- Hyperlipidemia
- BMI 30+
- 65+ years
- Family History of Hypertension
- Stress and tobacco usage/ alcohol >2 drinks a day
Smoking Cessation Sessions
Education classes for adults using Freedom Smoking Program evidence base curriculum. The program will be based in a community setting, in group or individually one-hour session once a week for a period of 4 to 8 weeks. This program is conducted by a Chronic Care Coordinator for adults who are ready to quit smoking.
